Liability Waiver
I understand and agree to the following:
Assumption of Risk: I acknowledge that participation in dance activities involves inherent risks, including but not limited to physical injury, falls, or accidents. I voluntarily choose to participate in activities hosted by Ahima Dance, LLC. dba Nashville Salsa Dancing and assume full responsibility for any risks, injuries, or damages that may occur as a result of my participation.
Release of Liability: I release and discharge Ahima Dance, LLC. dba Nashville Salsa Dancing, its owners, instructors, employees, contractors, and affiliates from any and all claims, liabilities, or demands arising out of or related to my participation in dance classes, workshops, events, or other activities.
Medical Attention: In the event of an injury or medical emergency, I authorize Ahima Dance, LLC. dba Nashville Salsa Dancing to seek medical attention on my behalf. I agree to be responsible for any costs associated with such medical care.
Fitness to Participate: I confirm that I am in good physical condition and have consulted a physician if necessary to ensure that I am fit to participate in dance activities. I will notify Ahima Dance, LLC. dba Nashville Salsa Dancing of any medical conditions that may affect my participation.
Media Release
Permission to Use Media: I grant Ahima Dance, LLC. dba Nashville Salsa Dancing the irrevocable right and permission to record, photograph, film, or otherwise capture my image, voice, and/or likeness during classes, workshops, events, or other activities.
Use of Media: I authorize Ahima Dance, LLC. dba Nashville Salsa Dancing to use such media for promotional purposes, including but not limited to social media, websites, flyers, and advertisements, without compensation to me.
Condition of Participation: I understand that checking the box to agree to this form is a condition of participating in Ahima Dance, LLC. dba Nashville Salsa Dancing activities. By checking the box, I agree to the use of my image and likeness as outlined above.
Agreement and Acknowledgment
By checking the box, I confirm that I have read and fully understand the terms of this Liability and Media Release Form. I agree to abide by the rules and guidelines set forth by Ahima Dance, LLC. dba Nashville Salsa Dancing and assume all risks associated with my participation.
I understand that checking this box constitutes my electronic signature and is legally binding, and that agreeing to these terms is required to participate in any activities hosted by Ahima Dance, LLC. dba Nashville Salsa Dancing.
